Dear Atty. Duran-Schulze,

Hi Atty. Could you please guide us on how and where we should file to add our son’s middle name to his PSA birth certificate, given that he was born in the US? Also, can my husband and I file on our son’s behalf since he is a minor? Thank you so much for your help!

Yes, you and your husband can file a supplemental report and affidavit of explanation on behalf of your son.

To add your son’s middle name to his PSA birth certificate, you should submit an affidavit indicating the entry missed in the document and the reasons why there was a failure in supplying the required entry. You will also need to provide supporting documents to verify your son’s name.

Since your son was born abroad, you should file the supplemental report with the Philippine Consulate where his birth was reported. But if your son is already in the Philippines, you can coordinate directly with the Department of Foreign Affairs, Office of Consular Affairs.
